Good Evening.

Please could someone show me a copy of thier consulation form to ensure that i am on the right lines as there are so many different one.

sorry to have to say this but lately their too many people on here asking for this kind of thing were you not shown one of these in your training search all the other threads about this you may find that a lot of geeks on here may say the same thing. i have a word document with loads of these on but i am not prepare to share it with any one esle as i did sell these on disk on ebay but found that people who was buying them were reselling them as their own work even though my name was on them and also emailed a few to numererous geeks on here. do a search and see what you come with designer nails have them on theri just copy and paste
 
From looking at your profile you are not qualified so should not have a need for client record cards, if you work on the public and something goes wrong ( theres lots that can go wrong with out training ) the public can sue you for thousands and more, im not saying this to be nasty im saying it for your own good, it is not worth taking the risk, my public liability insurance covers me for up to 2 million pounds so this shows how much someone can sue you for if they are that way inclined, please dont take the risk hun get some training and insurance, and take this as friendly advise nothing else :hug:
 
Its really good that you are thinking of a consultation forms - my assumption is that it is for nails?. It gives me the idea that you want to be professional whenever you've finished your training!

However my opinion is that it would have been more appropriate to perhaps give us an idea of what you were thinking of putting on your form. Then we would have been able to guide you and put you in the right direction.

To ask for a copy of someone's consultation form is not really fair, although there might be someone who would be willing to share theirs.

Its just so fulfilling to actually find the time to do this type of thing, then share it with others.

Go for it, 'draft' a consultation form, I'm sure you have the ability.
 
Looking at your other threads it seems you are qualified.. vtct.Perhaps you need to put this in your profile to avoid any confusion.

I would like to thank everyone for their comments. I am qualified and i will update my profile asap. (VTCT). I feel some of the comments are very harsh and and i am sorry if i offended anyone. I have a client form, but as i work from home i am on my own i do not get to meet many other nail techs,
as i work full time in another job
I do nails on the weekend and in the evening this one of the very few times i have.ever asked for help. I would be willing to help anyone should they ask me and i am sorry if other people have got ripped off in the past, but you shouldn't tar eveyone with the same brush.
i did get client forms on my course and i have purchased some the reason i was asking to have a look at someone elses was to make sure that mine were spot on.
I feel very hurt by the harsh comments!!!!!!!
 
dont feel hurt... im sure the geeks here didnt mean it in that way..

your profile may have lead to confusion... and there has been many times where someone here has asked questions and has taken advantage of the professionals on here to benefit themselves.. not to say that this is what you are doing.....

make sure you update your profile now and again... it will dismiss any misundertandings..

Here's mine...
I got it from the Designer Nails website quite a while ago, you will need to add Name and Address etc at the top of the form.
I saved it to Word and fannied about with it to get it looking how I wanted it ie. adding my logo at the top etc.

I must say that your declaration at the bottom is better than most I have seen on the pre-printed ones.

I always feel that the usual disclaimers are too harsh and could quite easily offend the customer as a cop out.

think your idea may follow on.
 
Cheers Theresa, but I can't take credit for it, these forms were designed by Gigi and were available to copy and paste from the old DN website.
 
I did the same thing, Just re worded it and made it a little girly

Hi I am exactly like you, a nail tech who has another job and I work from home doing nails in evenings and weekends. I wondered if you got anywhere with a disclaimer form which has worked for you? Am trying to compose one but need a wee bit of help. Many thanks in advance

Just to let you know, this thread is 10 years old & the poster you have quoted hasnt been on the site since last year.

If you pop your question into the search bar you will prob find liads of threads regarding disclaimers. They are not looked on favourably by proffessionals & the general view is that they are not be worth the paper its written on.

Maybe ask your insurer what they advise? Xx
