Client Record Cards

Just wondered if anyone has any reccomendations where i can buy record cards from ?

I already have Creative cards for my nail treatments but want something for the initial consultation, i have designed one myself but feel it is a bit to basic & am worried im not covering everything.

At my previous job, the spa had a form that was filled out with all medical details etc. Then for specific treatments ie. nails, facials we had the skincare ranges own branded cards to fill in.
I can't really remember all the different things that were on it just the layout & page size (A4) otherwise i would design another 1 myself.

I use the one from this thread... Consultation Cards
it's comprehensive enough and covers everything needed, apart from a date of birth.

Thats the same as the creative nails one that i use

If i had seen that before i would have used copy & paste !

Think i will use as template when run out !
