Colour consultation forms for mobile clients

Hi guys I'm setting up on my own and currently do a lot of colour correction and hair extensions, now I have a consultation form that clients must sign before we proceed with extensions and I'm thinking of getting some made for colour? I could then put the skin test date ect. Do u geeks think it's worth it or am I covering myself a bit too much? Thanks in advance

I just feel that ESP with colour correction they tend to not understand it and Suppose it looks professional x

What about using a client record card? That includes details of last skin test and you could add anything else like incompatibility test etc.

I use a consultation form on every colour I do, made my self.. And I
have boxes where they sign and myself so they understand what I've said and what is going to happen. And all the colours we have discussed. Also medication, previous hair treatments etc.

I don't give a copy to client just myself. So I print them out myself. I'll pm you x

I have a short form that i make clients sign to say i have performed a patch skin test and they have had no reactions but nothing as informative as yours Claire....I'd love to have a look at yours to see if its something that would work for me if thats ok?
