Consultation form

Hi Geeks
I am in the process of putting together a new and simpler consultation form that I am going to get printed professionally (with logo etc). I would be so grateful if you could let me know if you feel the form is ok or am I missing anything? I will be recording client records on the back of the form.
I am a mobile therapist covering a range of treatments, nails, shellac, massage, facials, waxing & tinting.

Hi there, I would just say, I think it should be 'advice' not 'advise', in that context. Hope that helps x
