Help with gel extensions!

This is the problem I keep encountering with my gel extensions. Can someone give my advice on how I can avoid this? Over filing? Product too thin? No apex?
 
I'd say no apex. The apex gives the strength in the stress areas where nails naturally bend.
 
If you look at the second picture the tech's nails have a definite stress apex, the gel nail is completely flat - and that split is classic stress area pressure split.
 
The nail does not have an apex, that would be the main culprit here. And to me it looks like the sidewalls have a tad too much product, which makes the nail kind of thick on the sides, but not where it should be thicker (around stress area). These nails don't need to be any thicker at the free edge and cuticle. There is definitely enough product on the nail, just not in the correct place ;)
