Mobile technicians: client cards

Hiya, Im really new to this and trying to get myself set up.

Does anybody have any examples of what client cards they use? just want to gage what information i need from the client in regards to their personal details and health

Thanks guys, that's really helpful. Alanya Mackie, are there any of those medical conditions that you would still do nail treatment on? ie. allergies, I guess it is dependent on what allergies? or a client with a pacemaker?

Both with allergies and pacemakers I'd get a gp note. Better to be safe than sorry. However depends what they're allergic to
