Client Record Cards - designed yourself

Hi all.

I've done a search and looked through old posts but things move on etc...new treatments, new ways of thinking...etc.

Those that make your own record cards what do you include to cover all aspects of beauty therapy? I mean you could have hundreds of questions!

I use fake bake cards for my spray tanning, dermalogica for my facials but everything else I just ask address, phone num & allergies.
I'd like to design a card that covers: nail services (man,ped,Minx,uv gel man)
Waxing Inc intimate.
Tinting,perming.
Massage
Spray tanning.

I will be doing skinbase microdermasbraison, anyone know if they supply their own consultation cards?

I have my own consultation cards.

It took a couple weeks for me to perfect them. I stick with the basic questions, allergies, medications, conditions, pregnancy, anything else health-related.

I have a release on the second page that clearly states that I hold no responsibility if anything goes wrong and they failed to tell me about something. Granted, release forms don't mean CRAP in court.

I've created a client consultation FORM that is two sided.
I wanted to cover EVERYTHING.
(including a media release for all the pictures that I take of my work).

Cancellation& Late Arrival Policy: Ongles Chez Victoria reserves the right to charge $20.00 for appointments cancelled or broken without 24 hours advance notice. Due to the potentially hazardous chemicals in the salon, please make other arrangements for your children on appointment days. Ongles Chez Victoria will not be held accountable for injury or accidents to, or caused by, unsupervised children.

Help yourself! I wouldn't have shared it if I had a problem with someone 'borrowing' from it.
I borrowed a bit from here and there, and added a few things of my own.
I remember when I started out... no such thing as Client Cards where I went to school
Even as a client, waaaaaaaaaay back, my tech didn't even take my phone number

And if anyone else thinks I missed something, PLEASE point it out LOL

Unfortunately, working from my living room can be challenging such as when I haven't finished folding the laundry and a teensy bit is left on the loveseat, orrrrr maybe my youngest left a floater in the loo and I missed it... orrrrr maybe my Boxer wants to give one of my dog-loving clients a "fido-facial" :o
I don't wear a uniform
BUT when I gets to the nitty-gritty, I do try HARD to rise above the local standards, that's for sure.

LOL my 'lot' (children & hubby included hahaha) behave well enough. They only offer fido-facials when invited hehehe Otherwise, they get told to 'stay' on the couch. At 1.5yrs old each, they're pretty good puppies. My boxer-cross (a rescue) suckers people in with her sad face (even the ones afraid of dogs) and my Rottweiler/Husky/Doberman suckers them in with her 'talking' (moaning/groaning type of growls, but not growls?) and makes a pitiful display too LOL
Children KNOW they are to stay out and play in their playroom OR outside, and they pretty much do so.
Hubby is a dear and offers clients coffee and tea (when he's home, if it's a wed/thurs/fri night, or sat during the day). Otherwise, it's just me and them alone (if you don't count my fur-babies lol)

I DO plan to expand into a salon of my own, just trying to figure it all out at the moment. Costs are prohibitive. I want 4 junior techs, an esthetician and a masseuse. That's my plan... or rather my 'hope' lol

We'll see how it goes.

Here's my furbabies LOL I'll miss them (as will most clients) when I get a proper salon.

My auntie has about 8 dogs in total and I was sat there stroking them one time and before I knew it I had nearly all of them trying to sit on my lap, dogs get jealous so easily bless them.
