Consultation forms, how may different types?

I do facials, acupuncture facials, massage, hotstone and aromatherapy and I'm trying to get away from having a different consultation form for each treatment.

How do youl do it?
What sort of treatments do you combine on the same form.

ie a form for body treatment, form for nails, form for facials..or do you have a general form that does it all?
I want to avoid having to do two forms when a client comes for a massage and a facial.

Do you always get a client signature before every treatment?

I have:

Massage
Waxing
Microdermabrasion
Facial / skincare
 
I have a record card which includes details and contra indications, allergies, meds plus questions related to waxing, tinting etc. checked each time. On the back is a grid with date, treatment details and space for a signature. I also have an extra consultation sheet for facials on which to record lifestyle fctors, skin analysis, condition details. I also record after care advice given.
