Hair extensions (balmain)

I trained with these a few years ago but never really got into it. I was in a mood last month had a massive clear out and threw all the paper work out.

Typically I now have a client who would like some hair extensions but this is only gonna be a one off thing.

Could anybody email me a consultation form that I can re do to make sure it fit these extensions. As I don't wanna buy a load more as I know I'm not goin to use them.

Just bumping this up again, am trying to write a consultaion and aftercar for out for my client and wondered if anybody would be able to email a copy of theirs that i can double check ive got everythin on mine.
