Own made client record cards

Forgive me, I know this question has been asked before! I can't seem to find the thread on here though (through my SG app)

I don't like any of the standard client record cards that are on sale through various salon warehouses. Most of my branded stuff is personalised and I would like to make my own client record cards. What do you Geeks think is important content for client record cards? They will be used for Makeup Artist services, hair styling (updo) service and Nail Tech Services.

I know the obvious ones would be client details, name, DOB, address, phone, GP details etc

Not too sure how to go about detailing the rest. thank yous in advance x

i make my own to tie in with my theme. I also have different cards depending on which treatment and then if they want a different treatment i make sure the relevant questions are covered if that makes sense. Im sure there's a better way to do it and just have one card but in a worrier and double check everything so i do it this way for my own peace of mind really. X
