Skin test forms!

Hi geeks, when you do a skin test in the salon what do you get the client to sign is it something you have written yourself or got off the Internet havnt got a clue what to do about one and I've googled it but nothing really comes up, could anyone share what they use, thanks xx

your local hair wholesalers normally sell ready made cards.....id definately buy some thann make any yourself as these look more proffessional and are more hardwearing.
You could try and get some online you tend to get more for your money x

Ok thank you, I've tried looking on line and I just can't find anything at all, the only thing I've ever seen in the wholesalers is just client record cards that record name and address and what colour you use on them never seen anything for skin test. Xx

your record cards normally have a box to tick saying if theyve had a skin test or not then your client signs at the bottom x

Right ok, didn't think that would be enough to cover me, just abit worried got a salon in a new area so with Xmas coming and getting busy and all clients are new everyone needs skin test so just wanted to make sure I'm covered as I keep hearing more and more about people having reaction. Xx

My college has a proper salon open to the public and the students are the 'stylists' when we have a client in for a skin test we just write it on their record card and have them sign it, then when they come back we note down whether or not they've had a reaction and they again must sign to confirm the information about the test is accurate, my college is hot on this sorta thing as its a training centre so im sure this will be fine for you to do too, but maybe ring your insurance company to double check, hth xxx
