Who fills out full consultation form for manicures/gel etc?

We just had a client refuse to fill it in and got quite angry so we couldn't do her treatment.

I asked if she doesn't want to fill it all out to just state name and any allergies and to sign it but she refused!

Client fills in the personal details, I fill in my recommendations and actions, client signs. If they are unable to complete for any reason, such as shakey hands, can't read/write I offer to fill it in myself and they can just sign it. She might not be able to read/write and doesn't want to say.

Thankyou yes didn't think of that. She was so stubborn though as to why we needed the info rather than that but you never know x
