Hi, I have done these new consultation forms and wondered if anyone has any advice. Would you add anything extra or change anything? Want to make sure we are covered for everything but without being too interrogating you never know if there is any comeback and what we are covered for if any issues. Also do I need the salon address on there at the bottom for example?
