Online consultation forms

Hello.

Does anyone use an online consultation form/record cards?

I'm just looking into going paperless with the new GDPR laws coming in, so wanting to find something suitable.

I have a Beauty & Holistic business, just me, that I run from home. So hoping to get something that is relatively easy to use and possibly adapt to suit all the treatments I offer.

I have looked at Phorest, but they offer their whole package which I'm not in need of as I already use a booking system that I'm very happy with. I have also looked at ClientForms and Formconnect... Not sure if anyone uses these?.

Any recommendations would be very gratefully received!! =)

I’m just curious, if you already use booking system, why do you need additional consultation form? Your current solution doesn’t provide that?

I was looking at these but have been unable to sign up, no one answers the phone and the person who answered Facebook message said they were on holiday.

It looks good but I would worry if there were any issues that I wouldn't be able to have them addressed in a suitable time frame.

They have now corrected the problem... so far so good. Very in depth.
