Tinting perming record cards

Hi Geeks,

so im due to start at the salon in May and when i went i did discuss with her client record crds and skin tests ect.

At the moment she just has lined aper with clients details on in her box. I was just wondering - i thought i could maybe make my own and put the same image ect on as i have on my business cards - thought would look more professional, any way i have tried to get a close up image on teh net but cant find one, can any geeks tell me the information which is on a lient record card for tintinga nd perming so i can make sure i have all the relevant info on my cards that i make?

I was thinking i could do three cards - Tinting - Perming and Services - this been for Cut/sets/conditioning treatments/ ect
