I spent 12 years running my own driving school.
It seems to me you need to do a "mock test" with another instructor, simulating from start to finish the whole test experience. These were invaluable for me, and also the pupil, and highlights areas needing work, but also are great from the point that it will build a lot of confidence in yourself.
Could also be, that no matter how well you get on with your instructor, familiarity breeds contempt? Just a thought.
